Bug Description
At the moment if you don't register a default scope controller, no scopes are on, therefore no flags can ever match. This is causing an unfortunate proliferation of people defining match-everything scope controllers.
We should make "default" always match, and clean up existing code to use it. Perhaps rather this should be done as part as having a registry pattern for scope controllers, with "default" always registered.

I think having a default scope would be useful, however the
profileration is 2 call sites, both of which I filed bugs on, and both
of which will be gone in a few days.
